I'm not sure of the name either, but look under the county that he/she is practicing in for a dental society and call them. Or you can look under Texas State Board of Dental Examiners.

Are you sure that he/she is on drugs, or is there a possible medical condition? Make sure before you make accusations.
I got this same response, and after writing back and fourth a few times, I got a little suspicious. I did some research ( like the rest of you) and found that there was nothing about the location, or any other contact location on either of the 2 websites.
So, I decided to write a e-mail back to tell them how much they stink. Here it is:

Thank you for getting up my hopes ( along with the hopes of others that have applied that have posted what a scam this is ALL over websites like Yahoo, and Google- good luck trying to get that bad reputation taken care of).

Also could mean you are not flossing enough in that area. If it bleeds when u floss that area needs some extra attention or so i am told.
I wouldn't volunteer the information but, if after they talk with your former employer and it turns out that they know, be honest. It was a mistake, which you now realize and such a thing will not happen in the future.
I wouldn't bash anyone, just the facts. If you don't get the job because of it, that is a consequence of your actions. If you have the merit to get a job without the reference, tell the new employer that they don't have permission to contact them citing personal reasons. See how that goes.

Yes you have a right to your 'own' medical records, and they should be provided within a reasonable period of time (i.e., not received a call requesting them immediately, the office drop everything for a file search, lining-up reproduction of the same, etc.).

If these are scores that you received before completing the Kaplan course, then you are fine. If you put 3 months into studying for it via Kaplan and finish your sciences, your scores should improve by at least 3-5 points.

Simmply contact them directly. You can find their phone number in the phone book. In my state, is is actually in a black box as the very first listing under "Dentists" in the Yellow Pages.
you may as well tell the truth, as the first answer so naively put it, there is no law against giving truthful references good or bad, as it is the medical profession they may be more inquisitive than if in another line of work. All you can say was that you had a lapse in judgment and have paid dearly for it

im 11 and i want to be an orthodontist when im older and what i’ve learned is another way to clean your teeth with out mouth wash is fill a glass or bottle with clean filtered water and then put toothpaste in it. then really dmix it in your mouth and spit it out.:) thats what i learned
