Much good luck on that one!! Most hospitals, dieticians and nutritionists follow the USDA food pyramid for meal planning. They do not take into consideration that each of us is individual and cannot eat all the foods recommended for diabetics.
I suggest that you consult an Endocrinologist and Certified Diabetes Educator in your area.
I have on record, that the doctor that admits me to hospital ask my daughter what foods to include on my plate and that I be allowed to administer all my own maintainance meds from my personal supply.
I cannot eat most of the carby foods on the list of "good for diabetics" foods! I would much rather see people watch the grams of carbohydrate per plate than a set number of calories.
If the hospital is participating in your plan, they can only bill you for the co-pay, deductible, and coinsurance.
Also, you could be billed for any non-covered/excluded charges. (If you were notified in advance that the service you were having was something that Medicare doesn't cover.)
I am pregnant with my first and I looked into several options. I opted for a hospital birth. During my research, I found that insurance rarely covers a home birth, and a midwife will not accept a first time mom with a home birth because of the possibility of complications. Basically, they want you to go through it first at a hospital before doing it at home. Complications the first time around tend to repeat themselves with a second pregnancy. I would recommend you give birth in the hospital this time, and if you have an uncomplicated pregnancy and birth you can consider a home birth next time. Good luck!
Doctors usually refer to whatever hospital they have privledges at, simply because it's a known facility. Don't you recommend places you know? "Kickbacks" don't usually come into play. It is not their responsibilty to know where your insurance's "preferred provider" is – it's YOURS. (It's impossible for a doctor or his staff to know the difference between the thousands of insurance plans they deal with – you only have one or two plans to deal with.) If you know of another location for certain things – it's your responsibilty to speak up and let the office know, as you found out the hard way.
All insurance plans try to save whatever money they can – why should they pay for something when you can? That's why the $200 copay for outpatient visits. Many people use hospitals for primary care instead of a regular doctor and therefore it costs insurance companies more money when people do this, and money doesn't grow on trees for anyone. (I know someone who went to the local ER – which is minimum $1000 a visit billed to the insurance – because her child was diagnosed with a heart murmur and she wanted to know more about it. This person could not understand why her insurance refused to pay for this visit. Ummm. Because that's why they have doctors in their network!!)
As you found out the hard way, which is sucky, it's the patient's responsibility to know their own insurance. Every single plan is different – there's no "common for all HMOs" – it's all based on individual employers and what they're willing to allow.

Yes, most hospitals will work something out. In desperation pay SOMETHING every month to keep it out of collections. Check with a social worker to see if there is any temporary help available in your state for medical costs. Good Luck.
You could try the city offices, the floorplans may be on public file from when the planning permission was granted to build the hospital.
But the best bet is to ask the hospital.
And then all you have to do is position the user inside the hospital, that is a far harder problem. GPS is worthless, even if it can get a position lock and the x/y positions aren't worthless it has no chance of getting the altitude accurate enough to tell you which floor you are on.
I don't meant to put you off but unless you've already got a method planned for getting the location information you should work that out before putting any effort into getting floorplans or writing a program to display the information.
